PET OWNERS :

• Please only fill out this form if you have a cardiology consultation booked with Pacific Coast Veterinary Cardiology. If not, please discuss a cardiology referral with your family veterinarian.

• Please take the time to complete the history form at least 2 days prior to the planned consultation. This will give me important details about your pet’s condition and will allow me to provide optimal recommendations. 

• To fill out the form you will need the following details: your pet’s respiratory rate when sleeping (please follow the instructions on how to measure your pet’s respiratory rate), a list of current medications and, if you have, a short video of any transient symptoms you may have witnessed at home (such as fainting episodes).
